The Transitional Care Management (TCM) program caters to patients recently discharged from hospitals or similar facilities. TCM entails a healthcare provider conducting a series of both in-person and telehealth appointments with the patient over a 30-day period. These appointments aim to maintain continuity in patient care, minimizing the likelihood of gaps and averting unnecessary hospital readmissions.

Beyond transitional care management, we also specialize in mobile urgent care, doctors’ house visits, and concierge medicine.

What We Do

Initiate contact within 24 hours of discharge to arrange your initial in-home visit

Conduct comprehensive medication review, assess home safety, and provide clinical evaluation

Engage in dialogue regarding your healthcare objectives and tailor a care plan to suit your individual needs

Offer specialized education programs and protocols tailored to facilitate your recovery process

Provide regular virtual check-ins from our nursing and/or therapy teams

Offer assistance in accessing community resources and scheduling follow-up appointments

Facilitate seamless transitions to inpatient and outpatient rehabilitation facilities as required

Ensure 24/7 access to an on-call nurse for any urgent concerns or inquiries
